At DAO we understand how important it is to have well-trained staff responsible for coordinating all the patient/consumer’s health care needs.  Care coordination is the centerpiece of individual and family access and continuity of optimal health care services

We implement a pioneering cultural and linguistic competence approach that goes beyond the one-dimensional instructional model. Our proven model has helped health care organizations, social service organizations and other community-based organizations achieve effective care coordination strategies.  Our model is backed by cultural and health science knowledge, and promotes cognitive learning (critical thinking about the benefits of intercultural communication) and affective learning (creating values and behavior that illustrate intercultural communication).

This program is designed for nurses, case managers, physicians, referral specialists, ad any other personnel with care coordination responsibilities.

Participants will effectively discuss, describe, and apply the following in their daily work:

Understand the philology, values, and conceptual frameworks of cultural competence.

Understand and apply care coordination framework. 

Understand and respond effectively to diverse belief system related to health and well-being. 

Use best practices for including cultural and linguistic competence in care coordination. 

Apply skills to conduct assessment of cultural and linguistic needs of a culturally diverse population. 

Apply knowledge and strategies in how to partners with network providers. 

Identify and address barriers in communication. 

Understanding cultural nuances and implementation of effective strategies to eliminate miscommunication and misunderstandings.
